The Ketogenic Diet – Is it time to switch?

myhealthygut keto wpThere are a plethora of blogs, articles and books promoting the ketogenic diet or “keto diet” for short term and health benefits. Celebrities swear by it, it seems like everybody’s doing it – is it time to jump on the bandwagon?

The ketogenic diet is a therapeutic diet that has been used successfully for almost 100 years to treat seizure disorders that do not respond to traditional medications. We are seeing a resurgence of this diet to manage a variety of medical conditions, including obesity, diabetes and cancer.

What does the keto diet look like?
The keto diet is a high fat diet. Carbohydrates are restricted to less than 5-10% of total calories (less than 50 grams/day), protein intake is 15-20% and fat intake ranges from 75-80% of total calories. In order to stay in ketosis, most individuals consume around 20 grams of carbohydrates daily. In practical terms this means a maximum of 1 slice of bread or ½ cup of pasta or rice daily. Bring on the butter, cream and oils!

How does the keto diet work?
The human body is fascinating! When we have access to carbohydrates in our diet, we are able to digest and process the carbohydrates into glucose. Glucose is the primary fuel source for our body. When the amount of glucose decreases in our body, our body needs to find an alternate fuel source and that is fatty acids. Ketones are a product of fatty acids. The shift to using ketones as the primary fuel source (instead of glucose) is thought to be responsible for the health effects, although research is ongoing.

Is there research to support this diet?
The therapeutic benefit of the keto diet in individuals that have epilepsy is well-supported by scientific evidence. In fact, when children follow the keto diet under strict medical guidance the effects of the diet are comparable to modern anti-seizure drugs 1 . The Italian Society of Gastroenterology supports the use of the diet in individuals with severe obesity that have been unsuccessful with conventional medical treatment 2 . Unfortunately, there is no research on the use of the keto diet in individuals living with digestive disorders such as celiac disease, irritable bowel syndrome or inflammatory bowel disease.

Are there risks to following the diet?
The main side effects reported as a result of following this diet are gastrointestinal symptoms, including vomiting, constipation and diarrhea 1 . These side-effects can last days or weeks – often referred to as having a “keto flu”. Individuals living with digestive disorders need to be extra cautious about ensuring appropriate nutrient intake, as following a keto diet incorrectly can cause nutrient deficiencies and compromise health. Not surprising, the most challenging part about following a keto diet can be sustaining the diet due to its restrictive nature.

An emerging concern is the impact of the keto diet on the gut microbiome. Unfortunately, there is a lack of available research in humans. It is however, well supported in the literature that a diet rich in omega-6 fats (e.g., corn oil, soybean, safflower, sunflower oils and ultra-processed food), red meat but lacking fibre-rich foods is detrimental to the gut microbiome. So, ensuring the inclusion of healthy oils such as olive and avocado, along with more fibre rich plant-based proteins while following a keto diet may counteract some of these negative effects, although this still needs to be studied.

What is the bottom line for those living with digestive disorders?
Unfortunately, there is no evidence at this point to support the use of a keto diet to help manage digestive disorders. Because there is so little information on how the keto diet impacts the gut microbiome, following the diet could be quite risky. Experts in the field would say “for now, a well-balanced diet, tailored to the individual to target specific disease requirements, is the best option for people (living) with gastrointestinal conditions” 3 .
